You will receive a welcome email containing the following forms: 

  • New Patient Registration Form
  • Patient History Form
  • Allergy History Questionnaire
  • Review of Systems

Fill out these forms beforehand and bring them to the office for your appointment. This can be accomplished by printing them out and writing your information or by using a website or iPad to manually type your answers on the PDFs.

What to expect at your first visit

Please fill out the aforementioned paperwork and initial history and submit. Bring your insurance card and any medical records or tests related to your visit (or have your medical office fax to us).

Your insurance will have a list of participating providers—please double check that you see Dr. Fowler listed there. Know your insurance! Out-of-pocket costs for first visits with allergy testing can be around $1,000. A high-deductible plan may leave you with a bill of $500 – $1,000 even if we are in your network.

Co-pays are due at the time of service. If your insurance requires you to have a referral, you must arrange that prior to your appointment.

Out of consideration for other allergy patients and our providers, refrain from wearing any perfumes, colognes or scented products. If you have indoor pets, please wear fresh clothing. Additionally, we do not allow food in the office to best maintain a safe and clean space for all our patients.

Our office will need to schedule allergy testing and make the antigen at a future appointment.

For new patients, you will always come to our Columbia location. We will not test you on your initial visit if you have commercial insurance.

Please arrive at least 30 minutes early for your appointment if the new patient paperwork cannot be completed prior to your appointment.

Information for testing appointments

If you are taking an antihistamine or beta-blocker, you must be off it for 5 days prior to your appointment. If you are currently on allergy shots or drops, you will need to be off those for 7 days.
